Post hospitalization rehabilitation is an integral phase of healthcare. Older adults have higher risk of readmission due to inadequate rehabilitation post discharge. Built on the team’s preliminary work in the area of longitudinal monitoring of mobility, diet and nutrition, and exercise, the project will develop an integrated at home rehabilitation solution aimed to improve the post-hospitalization care to older adults at home. The Project will improve the care provided to older adults as well as the quality of life to them and their caregivers. It will also benefit the healthcare system by potentially allowing earlier discharge, increased safety, and improved patient/caregiver satisfaction. The technology and associated healthcare products and services will lead new socio-economic benefit to the private sector through commercialization.
